The ID card contains:
*Pet name
*Breed
*Spayed/Neutered
*Birth date
*Rabies tag number
*Microchip number
*Vaccine history
*Your contact information 
*Our practice information
*After hours emergency information

*You will need to be signed up for your pet portal in order for us to create an ID card for your pet. Our pet portal is available on the home page of our website on the left hand side.
    

*In order for your pets picture to be on the ID card it must be up uploaded onto your pet portal first.

If your pet is due for vaccines in the near future, please wait before ordering your ID card so the vaccines will be current on your ID card.
